Job Description
As a leading home health staffing agency, we are seeking compassionate and dedicated Home Health Aides to join our team for Per Diem visits, providing high-quality care to patients in the comfort of their homes. In this role, you will assist with essential daily activities, including bathing, dressing, grooming, and mobility assistance, ensuring that patients feel comfortable, safe, and supported. You will also provide valuable companionship, assisting with light housekeeping, meal preparation, and other non-medical tasks that enhance patients’ quality of life. Your care will play a crucial part in helping patients maintain independence and well-being.

Key Responsibilities
- Deliver Compassionate Care: Assist patients with daily activities such as bathing, dressing, grooming, and mobility, ensuring a safe and comfortable environment.
- Provide Personal Support: Offer companionship and emotional support to patients, promoting mental and emotional well-being.
- Light Housekeeping & Meal Preparation: Help with light housekeeping duties, such as tidying up living areas, laundry, and preparing nutritious meals.
- Collaborate with the Care Team: Communicate with physicians, caregivers, and family members to ensure care plans are followed and adjusted based on patients’ needs.
- Monitor Patient Health: Observe and report any changes in the patient’s physical or mental condition, ensuring timely updates are communicated to the appropriate team members.
- Maintain Documentation: Accurately record patient activities, progress, and any changes in condition, completing required reports in a timely manner.
- Uphold Confidentiality: Maintain patient confidentiality in accordance with HIPAA guidelines and privacy standards.
- Comply with Regulations: Perform duties in alignment with agency policies, procedures, and all relevant local, state, and federal regulations.
Skill & Experience
- Completion of Home Health Aide training program.
- Valid Home Health Aide certification (required).
- Compassionate, reliable, and patient-focused attitude.
- Ability to follow care plans and assist with daily living activities.
- Strong communication skills and the ability to work well with patients, families, and healthcare team members.
- At least 1 year of experience in home health or caregiving preferred but not required.
