As a trusted home health staffing agency, we are seeking compassionate and skilled Registered Nurses (RNs) to join our team for Per Diem hospice visits, providing exceptional, compassionate care to patients in their final stages of life, within the comfort of their homes. In this role, you will follow the care plan provided by the attending physician to administer essential treatments, including pain management, symptom control, and end-of-life care. You will provide emotional and educational support to both patients and their families, helping them navigate the complexities of hospice care. Through follow-up visits, you will monitor patient progress, adjust care plans as needed, and ensure that patients are receiving the appropriate support during this critical time.

Key Responsibilities
- Deliver Compassionate Hospice Care: Provide exceptional, empathetic care to patients during home visits, ensuring comfort and dignity during the end-of-life journey.
- Pain and Symptom Management: Administer prescribed treatments and medications to manage pain and alleviate symptoms, while continually assessing the patient’s condition.
- Family Education and Support: Offer thorough education to families on end-of-life care, managing symptoms, and providing emotional support throughout the hospice process.
- Collaborative Care: Work closely with physicians, caregivers, and other healthcare professionals to ensure a holistic, compassionate approach to care, addressing both physical and emotional needs of the patient.
- Adjust Care Plans as Needed: Regularly monitor patient condition and provide follow-up care, adjusting the care plan as necessary to meet changing needs and ensure comfort.
- Maintain Open Communication: Keep physicians, caregivers, and family members informed about changes in the patient’s condition and ensure any necessary adjustments to the care plan are made.
- Documentation & Compliance: Complete all required documentation promptly and accurately, ensuring compliance with regulatory standards and maintaining patient confidentiality.
- Adhere to Regulations & Agency Guidelines: Execute duties in strict accordance with agency policies, procedures, and relevant local, state, and federal regulations, upholding the highest standards of care and confidentiality.
Skill & Experience
- Registered Nurse with a valid California RN license.
- At least 1 year of experience in hospice, palliative care, or home health nursing.
- Strong understanding of end-of-life care principles, including pain and symptom management.
- Ability to provide emotional support to patients and families during a sensitive time.
- Excellent communication skills and ability to work effectively as part of a collaborative team.
