Job Description

As a home health staffing agency, we are seeking compassionate and skilled Registered Nurses (RNs) to join our team for Per Diem visits, providing exceptional care to patients in the comfort of their homes. In this role, you will follow the physician’s care plan to administer essential treatments, including medication management, wound care, and comprehensive patient assessments. You will also play a key role in educating patients and their families on proper medication usage, self-care strategies, and the safe use of assistive devices and medical supplies. Through follow-up care, you will monitor patient progress and make any necessary adjustments to the care plan to achieve optimal health outcomes.

Key Responsibilities

  • Deliver exceptional patient care during home and facility visits, ensuring a comfortable and effective treatment environment.
  • Administer prescribed treatments, medications, and nursing interventions to patients and families, tailored to their unique needs in home or facility settings.
  • Provide thorough education to patients and families on proper medication usage, self-care techniques, and the safe use of medical supplies and assistive devices.
  • Implement nursing interventions in alignment with the care plan established by the Attending Physician and Home Health Agency.
  • Maintain open communication with physicians, caregivers, family members, and the care team, ensuring any changes in patient condition are promptly addressed and care plans are adjusted as necessary.
  • Stay up-to-date with relevant government regulations, healthcare standards, and industry guidelines, ensuring compliance in all aspects of patient care.
  • Accurately document patient information and treatment details, completing required reports in a timely manner.
  • Uphold patient confidentiality at all times, adhering strictly to HIPAA regulations and privacy standards.
  • Execute all duties in accordance with agency policies, procedures, and applicable local, state, and federal regulations.

Skill & Experience

  • Valid California RN License
  • At least 1 year experience in either hospital or home health setting.
